Do pikolinos run large or small?

Love the style, but this does indeed run small. Although I wear a 9.5, I thought I would be safe ordering the 40 (which runs to a 10), but it feels way too tight. Exhanging today. As far as comfort, the insole has a nice amount of cushioning.
